• PD-L1 (imperfect) gold standard to give IO.
• TMB expensive and immature to give IO.
– Better than PD-L1? Better TMB & PD-L1? Threshold? Platform?
– High TMB predictive in PD-L1 neg. tumours
• LIPI, easy score (LDH/neutrophils) to give IO.
• Some groups of patients not to give IO.
• Immunotherapy probably does not fit all patients.
